Attendance

Attendance will be taken each day of the program. When this program is facilitated virtually (via Zoom), Sharnise Hendrick and/or an administrative staff member at Lotus Embodied Counseling, will record attendance based upon attendee login to the training.The host will notified of your arrival and departure.

For all in-person programs, attendance will be taken at the beginning and end of each day of the program. Please note that Sharnise Hendrick is required to maintain records of all attendees for five (5) years. These records will be kept in compliance with HIPAA.

CE Certificate

Upon completion of the program evaluation form, participants will receive their CE certificate within one calendar week (7 days). The certificate will reflect the name the participant used to register for the program. Please use the name in which you want reflected on your registration form. Certificates will be distributed via email.

Program Evaluation

Participants will be emailed a link to a Google Form directing them to complete a program evaluation form. The link will be emailed to participants on the final day (Day 2)of the program. Upon completion, Sharnise Hendrick will be notified, and the participant will receive their certificate of completion within one calendar week (7 days).

Evaluations are taken seriously and used to support productive changes in the program for future offerings. Specific information included in your feedback is appreciated and valuable to this program.

Grievance Policy

Sharnise Hendrick and Lotus Embodied Counseling welcomes feedback and the opportunity to address program participant grievances. Grievances, continuing education approvals, refunds, and any other questions or concerns should be submitted via the grievance form (see end of this document for the form). This form can be emailed or mailed.

Any grievances are discussed at a weekly owner’s meeting, consisting of Sharnise Hendrick and Caroline Falk, co-owners. Each maintain the commitment to take all grievances seriously and explore strategies to support resolution and/or adaptation to trainings to better support future participants.

Any requests will be responded to within five business days of coming to a decision regarding that grievance. Depending on the nature of a grievance, the discussion may be ongoing. Participants also have a right to issue a complaint with any of the continuing education approval sources connected to our trainings if you feel your rights as a learner have been violated or disrespected.

Refund & Cancellation Policy

If cancelling prior to two weeks before a scheduled event, trainees are entitled to a full refund less a 5% processing fee OR the option to apply the dollar amount paid towards a future training with Sharnise Hendrick and Lotus Embodied Counseling.

If cancelling less than two weeks before a scheduled event, trainees are entitled to a 50% refund, OR the option to apply the dollar amount paid toward a future training. Only medically documented reasons or family deaths are considered as an exception to this policy. No-shows are not eligible for a refund.

In the case of inclement weather, should we cancel the training, your registration would simply be moved to our rescheduled date. If the rescheduled date isn’t available for you, we can offer you another date or issue a refund.

If you terminate your training or need to leave it early for any reason, you are not eligible for a refund. Attendance at the entire training is required for our program to remain compliant with CE credentialing bodies. All other situations will be handled on a case-by-case basis. Replacement Documents Policy

Replacement documents (certificates of completion, CE verification, wall certificates, etc.) can be replicated in the event they are lost or unavailable. We reserve the right to to charge an administrative fee of $10.00 per document for replacement if the request is over 30 days from when the document was originally issued.

Social Media Policy & Contacting Sharnise Hendrick

No advice related to diagnosis or treatment can be given to people who are not direct clients or consultees, and never over social media. Advice of this nature must be kept in client or consultation records according to the standards applicable to those relationships, including relating to privacy and client confidentiality. Participation in or on these social media platforms (including Private Messaging) does not create a professional relationship between the participant and Sharnise Hendrick or the individual and Lotus Embodied Counseling.

Please refrain from using Private Messaging on any of Sharnise Hendrick’s or Lotus Embodied Counseling’s pages (personal or professional) to ask business questions, including but not limited to requests for treatment providers and professional consultation questions. Due to the high volume of emails, please allow 48 hours before sending a follow-up email.

Sharnise Hendrick attempts to be as interactive as possible, yet has scheduling and personal boundaries that will be set and maintained. If email communication becomes an indicator that professional consultation may be a better fit, Sharnise Hendrick will set this limit and require you to step into the realm of professional consultation that will be a separate fee and agreement. Messages to social media inboxes may not receive a direct response.

Threatening or abusive behavior towards Sharnise Hendrick, Lotus Embodied Counseling, or any staff/contractors affiliated, will not be tolerated and will result in an immediate block or ban from all electronic and social media contact. Abusive behavior will be reported to the appropriate source(s). Harassment, racist behavior, or hate speech will also result in an immediate ban.

All venues used for in-person trainings are ADA-compliant. For both in-person and online trainings, if you require assistance, Sharnise Hendrick and Lotus Embodied Counseling are willing and committed to making reasonable accommodations.
